What is Dental Scaling?

Scaling is a non-surgical, therapeutic procedure that involves removing plaque (a sticky film of bacteria) and tartar (calculus – hardened plaque) from the tooth surfaces, both above and, most importantly, below the gumline.

  • Plaque: The soft, sticky film you can remove with brushing and flossing.

  • Tartar/Calculus: Once plaque hardens into tartar, it cannot be removed by brushing. It acts as a constant irritant and breeding ground for bacteria, leading to gum inflammation and disease.

Why is Scaling Performed?

It is the primary treatment for gingivitis (early, reversible gum disease) and periodontitis (advanced gum disease). Signs you might need scaling include:

  • Red, swollen, or tender gums

  • Gums that bleed easily during brushing or flossing

  • Persistent bad breath (halitosis)

  • Receding gums or teeth that look longer

  • Pus between the teeth and gums

  • Loose teeth (a sign of advanced periodontitis)

The Scaling Procedure: What to Expect

A scaling appointment is typically longer than a regular cleaning and is often paired with root planing.

  1. Assessment: The dentist or hygienist will use a probe to measure the depth of the pockets between your teeth and gums. Deeper pockets indicate gum disease.

  2. Anesthesia: Often, a local anesthetic is used to numb the areas being treated to ensure complete comfort, as the cleaning goes below the gumline.

  3. The Cleaning:

    • Ultrasonic Scalers: A tool that uses gentle vibrations and a water spray to break up and flush away larger pieces of tartar.

    • Manual Scalers (Curettes): Fine-handed instruments are then used to meticulously smooth the tooth and root surfaces (this is the root planing part). This removes any remaining debris and creates a smooth surface that makes it harder for plaque to re-adhere.

  4. Polishing & Fluoride: The teeth are often polished to remove surface stains, and a fluoride treatment may be applied to strengthen the teeth.

Aftercare and Recovery

  • Sensitivity: Teeth may be sensitive to hot and cold for a few days.

  • Soreness: Gums may be tender.

  • Healing: Patients will notice a significant reduction in bleeding, swelling, and bad breath as the gums heal over the next week or two.

  • Follow-up: A follow-up appointment is crucial to check healing and pocket depths. For periodontitis, ongoing maintenance cleanings every 3-4 months are usually recommended.
